Fast food restaurants that don't offer a burger without cheese as a menu item.
For example, "Quarter Pounder with Cheese" is a menu item at McDonald's, but "Quarter Pounder" is not. Similarly, at Wendy's, other than the "Jr. Hamburger," all of the menu items have cheese (e.g., the "Dave's Double" defaults as having two patties and two slices of cheese).
Of course, I can't order a "Quarter Pounder with no cheese, but there are two problems with that. First, the average fast food employee seems to have trouble with the concept of ANY "special" orders, but the concept of no cheese on a "Quarter Pounder with Cheese" seems to be especially problematic.
Second, if I want EXTRA cheese on my burger, Wendy's is going to charge me 50 cents per slice. However, if I want NO cheese, Wendy's isn't charging me a dollar less (50 cents per slice that I'm not getting). By contrast, places like Five Guys and In-n-Out price their burgers based on what you get on them and don't charge you for cheese you aren't getting.
