Almost certainly Maiden at this point. Don't keep a list, but in the 15 or so range are Metallica, Dio and Megadeth. It's strange, I'm not even all that crazy about Megadeth, but they just popped up a lot in various package deals and on support slots. One of a few bands I've seen more times than I would have imagined.

In answer to OP's question, there are a few bands on the always see at all costs list. Plenty on the always see when they come to town list. Plenty that will warrant a trip to Houston or Austin. I've probably seen DT 10+times, and that's largely because of some Houston and Austin gigs. There are also some bands that get downgraded. I have no interest in what Rush does on stage anymore. Priest are probably off the list as well. In both cases Rob and Ged can't hang anymore and the setlists bore me. DT is a Dallas only gig nowadays. Just not all that interesting a show anymore.

After that; Opeth, Anathema, Leprous and Dream Theater with 4 each.

Anathema and Leprous with 4 each.

I'm incredibly jealous right now. The one (short) Leprous show that I got to experience was awe-inspiring. I'd love another chance to see them. Anathema too of course, as they've steadily worked their way into being my favorite band these last couple years.

I don't have the exact numbers at hand but I've seen Dream Theater and Toto each around 10 to 15 times, with Toto slightly in the lead. I've seen a lot of other bands but none come close. Most bands I've seen only one or two times, a few about five times.
Must've been Kwyji sending all the wrong songs.   ;D

I'm not really a concert goer so aside from this certain local Swedish band I've seen about 5-6 times, I guess DT with a grand total of two times gets the spot :lol

Queensryche: 19 times (last time was in 2009)
Rush: 18 times

side note: in addition to the 47 times I saw MP with DT, I also saw him perform at 20 other shows in LTE, G3 (with JP), Transatlantic, Neal Morse, Adrenaline Mob, Cygnus and the Sea Monsters, Amazing Journey, Yellow Matter Custard, PSMS, Flying Colors and The Winery Dogs.

As for the greatest distance I've traveled, in the past it would've been from SoCal to the New England states (and then road tripping to several shows while there) but now I'd say internationally (3600+ miles one way).
As a basic rule, if you hate it, you must solely blame Portnoy. If it's good, then you must downplay MP's contribution to the band as not being important anyway, or claim he's just lying. It's the DTF way.

I've seen Dream Theater 4 times. Except for that the most times I've seen any band/artist is twice, and that has happened with Maceo Parker, Parliament/Funkadelic, Mezzoforte and Kamelot.

Dave Matthews Band is one.  I've seen them five times, with four of those times coming in two different back-to-back nights they did here back in the early 2000s, and the set lists were completely different each time.

Perhaps, but in this instance, that's besides the point. My point is that I used to go see them numerous times on each tour. Now I don't, and that's largely due to the lack of rotating setlists.

And KevShmev's example (one post up) is another example of a band who (still) rotates setlists. I've never been to any of his shows, but from what I understand, a significant part of his audience will go see multiple due to the jamming and the changing setlists.

Another band that I can think of that I believe change things up from show to show (besides jam bands) is Pearl Jam, who also have a strong following that catch multiple shows. I wonder how many of them would if they stopped changing things up from night to night?

I agree, that Pearl Jam would loose multiple viewings on their tours.
